Golden Freight Claim

Report freight damage, attach the photographs and the bill of lading, and Stellor will take it from there.

Stellor Medical reviews every freight claim before it reaches the Golden freight claims team.

Freight damage should be reported within 48 hours of delivery whenever possible. Please include photographs of the damage, and a photograph of the signed BOL if it is available.

Please do not include patient information. This form is for equipment, account and logistics detail only. Nothing here should identify a patient.

Required for Golden Technologies claims.

The date it came off the truck, not the date the damage was found.

Was the BOL signed free and clear, or was damage / an exception noted?

Answer it as it was actually signed. A claim is not automatically refused because the BOL was signed clear, and a wrong answer here is found later and costs more than the right one did.

Has the manufacturer already been notified?

If you have already spoken to them, say so and Stellor will not duplicate the report.

One per line. Leave blank for freight that genuinely carries no serial -- parts, cartons, accessories. Do not invent one.

What is damaged, and how. Equipment and freight detail only -- do not include patient information.

Photographs of the damage are strongly recommended -- they are what Stellor reviews the claim against, and a claim that shows the damage moves faster. A claim without them is still accepted and still reviewed.

Attach several -- the damage itself, the carton, and the unit in place. JPG, PNG, HEIC, WEBP or PDF, up to 12 files in total across both boxes, 12 MB each.

Strongly recommended when you have it. If the BOL is not to hand, submit the claim anyway and send it on when you find it.
